The calendar below will allow you to book our first initial video visit. In order to book the appointment, you will need to enter credit card information and submit payment. Please remember the patient has to be present for this appointment and if the patient is under 18 years of age, the guardian/parent will also need to be present.
If you have not already, please register in our patient portal and complete the required documents prior to our first visit. You should receive an email to sign up for the Charm patient portal. The patient portal will allow you to send messages securely to me and allow you access to your medical records. It is how you will also securely send your previous health records to me to review before our appointment and book your future appointments.

Of note, the time zone defaults to mountain (To adjust your time zone in spring: for TX add 1 hour to mountain time; FL add 2 hours; in the falll: TX add 2 hours to mountain time, FL add 3 hours) .
